1. Original 6 months Valid passport.
2. One visa application form online filled with Appointment Date.
3. Two colour photographs 35m by 45mm 80% face should be seen white back ground
4. Covering letter on company letter head addressing to the Poland consulate Mumbai with company Profile,
5. Invitation letter with the passport details of the Applicant addressing to the Poland consulate Mumbai
6. Overseas Medical Policy
7. Return Air ticket.
8. Insurance As per Travel Dates.
9. Passenger’s personal IT last 3 years
10. Salary slip last 3 months
11. Passenger’s personal bank statement last 6 months
12. Appointment letter copy.
13. Company Registration Copy
Tourist Visa :
1. Original 6 months Valid passport
2. One visa application form online filled With Appointment Date.
3. Two colour photographs 35m by 45mm 80% face should be seen white back ground ,
4. Covering letter on company letter head addressing to the Poland consulate Mumbai ,
5. Hotel confirmation from Poland.
6. Overseas Medical Policy
7. Air ticket,
8. Passenger’s personal IT last 3 years
9. Salary slip last 3 months,
10. Passenger’s personal bank statement last 6 months, If Employee than required Leave letter from company.
11. If Director or Proprietor Then Memorandum of the Company or Gumasta License or Any Proprietorship Proof.
Employment visa :
1. Original 6 months Valid passport
2. One visa application form online filled with appointment Date
3. Two colour photographs 35m by 45mm 80% face should be seen white back ground
4. Covering letter on company letter head addressing to the Poland consulate Mumbai
5. Original Work permit approval from Poland
6. Invitation letter from Poland company with Passenger’s ppt details addressing to the Poland consulate Mumbai
7. Work experience certificate
8. Qualification certificate
9. Resume copy of applicant
10. Overseas Medical Policy.
W.E.F 13th December 2019 Poland VFS Is Starting In Mumbai So Now All The Visa Applications We Have To Submit In The VFS With Prior Appointment Instead Of The Consulate, All The Applicant Have To Come Personally For Submission And Biometrics At VFS With Prior Appointment.

This Biometric Data Will Be Valid For 5 Years , The Travellers Who Have Already Given Their Biometric
